Breast reconstruction is one way to regain the self-confidence and womanly figure marred by cancer. While breast reconstruction is an extensive surgery, it can often be done in conjunction with a mastectomy. Depending on your natural breast size and the amount of excess muscle, skin, and fat tissue in other key areas of your body, there are several different types of reconstructive surgery from which to choose. Dr. Wittpenn is adept at performing the three primary forms of breast reconstruction: tissue expansion, TRAM flap, and Latissimus Dorsi flap.
Tissue expansion is a type of breast reconstruction that involves the gradual stretching and expansion of the skin around the removed breast. This is accomplished by inserting a tissue expander—a balloon-like device that looks similar to an unfilled breast implant—that is injected with fluid over the course of several months. As the skin slowly expands, it becomes more and more prepared to accept a breast implant. The final step in the tissue expansion process is the replacement of the expander with a breast implant, similar to the type used in breast augmentation procedures at our practice near Tyler, Texas and Shreveport, Louisiana. If the permanent tissue expander is used instead, then there is no second surgery involved as the fully-filled expander now serves as the implant.
Pedicled TRAM flap breast reconstruction is one of the most popular forms of reconstructive surgery. The procedure involves using a section of the patient's TRAM muscles, or abdominal muscles, to reform the breast. Dr. Wittpenn will move an oval-shaped section of skin and the underlying abdominal muscle, sliding it into place under the removed breast area. Unlike a free TRAM flap, the pedicled TRAM flap tissue remains connected to its original blood supply. Once the flap is in place, the tissue is shaped into a more natural-looking breast and the incisions are closed. This surgery is unique because the patient ends up with a tummy tuck of sorts—after the new breast is formed, the skin and tissue around the removed TRAM muscle is lifted and tightened, slenderizing the waist-line.
A latissimus dorsi flap is similar to TRAM flap reconstruction in that it too uses the patient's own muscle, skin, and fat tissue to reform the most natural-looking breast possible. Rather than using an abdominal muscle, this form of breast reconstruction involves the use of a section of the latissimus dorsi muscle - the back muscle that lies beneath the shoulder and behind the armpit. During the procedure, Dr. Wittpenn will move an oval-shaped section of skin and the underlying latissimus dorsi muscle around to the front of the chest. This type of breast reconstruction is a good option for women with medium to small-sized breasts because there is so little body fat in this area on the back.
